Registered Nurse

Summary of Responsibilities

The Registered Nurse with meet with patients of all ages in their residence for physician ordered care including wound care, medication management and administration, and education on the patient’s diagnoses. They will use their knowledge and critical thinking to communicate with caregivers, physicians, clinical supervisors, and healthcare team members appropriately about changes in patient’s condition and updates to the plan of care. The one-on-one setting ensures that the Registered Nurse can quickly connect with our patients to guarantee patient dignity during their recovery and make a positive lasting impression.

Qualifications

  • Graduate of an accredited Bachelor of Nursing or Associates of Nursing program.
  • Currently licensed as a Registered Nurse in the state of Idaho or other compact state.
  • Two (2) years of registered nursing experience in community/home health or medical/surgical experience is preferred.
  • Obtain and maintain current BLS CPR.
  • Capability to work independently and manage time effectively while
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
  • Access to a well working automobile for traveling to patients for care.
  • Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.

Licensed Practical Nurse 

Summary of Responsibilities

The Licensed Practical Nurse will meet with patients in their place of residence for physician ordered care including wound care, medication management and administration, and education on the patient’s diagnoses. They will use their knowledge and critical thinking to communicate with physicians, clinical supervisors, and healthcare team members appropriately about changes in patient’s condition to get the plan of care quickly and efficiently updated. The one-on-one setting ensures that the Licensed Practical Nurse can quickly connect with their patients to guarantee patient success on their recovery back to independence and make a positive lasting impression.

Qualifications

  • Graduate of an accredited Associates of Practical Nursing program.
  • Currently licensed as a Licensed Practical Nurse (LPN) in the state of Idaho or other compact state.
  • Two (2) years of registered nursing experience in community/home health or medical/surgical experience is preferred.
  • Obtain and maintain current BLS CPR.
  • Capability to work independently and manage time effectively while

Certified Nursing Assistant 

Summary of Responsibilities

The Certified Nursing Assistant (CNA) is responsible for following care instructions prepared by the Registered Nurse or other appropriate professionals who are responsible for the supervision of the CNA. This includes assisting the patient in bathing, dressing, and transferring. The CNA is responsible for observing patients, reporting, and documenting these observations and care performed. They will be assigned in a manner that promotes the quality, continuity, and safety of a patient's care. Qualifications

  • Meets the training requirements in accordance with state and federal laws.
  • At least 18 years of age
  • One year (1) of appropriate experienced as a Certified Nursing Assistant in home health preferred.
  • Obtain and maintain current BLS CPR.
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
  • Access to a well working automobile for traveling to patients.
  • Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.

Physical Therapist

Summary of Responsibilities

The Physical Therapist will use their professional skills to assess and evaluate the patient and develop an individualized plan of care in coordination with the patient's caregiver, physician, and other home health staff. The goal of the Physical Therapist will be to help the patient set goals based on their health history and personal concerns that will improve their balance, and gait while improving their strength, range of motion, and endurance safely to return to independence in the home setting. The one-on-one setting ensures that the PT can quickly connect with our patients to guarantee patient safe recovery and return to independence while making a positive lasting impression.

 

Qualifications

  • Possess a master’s or doctoral degree in physical therapy from a nationally accredited school with board approved curriculum.
  • Licensed to practice as physical therapist in the state of Idaho.
  • Two years (2) of appropriate experienced as a physical therapist (PT) in community/home health or clinical experience preferred.
  • Obtain and maintain current BLS CPR.
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
  • Access to a well working automobile for traveling to patients to provide care.
  • Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.

Physical Therapist Assistant

Summary of Responsibilities

The Physical Therapist Assistant will use their professional skills to assist and monitor the patient’s progression based on their individualized plan of care. The goal of the Physical Therapist Assistant will be to help the patient progress towards their goals and plan of care. This includes improving balance and gait while improving their strength, range of motion, and endurance safely to return to independence in the home setting. The one-on-one setting ensures that the PTA can quickly connect with our patients to guarantee patient safe recovery and return to independence while making a positive lasting impression.

 

Qualifications

  • Possess a degree in physical therapy assistant from a nationally accredited school with board approved curriculum.
  • Licensed to practice as physical therapist in the state of Idaho.
  • Two years (2) of appropriate experienced as a physical therapist assistant (PTA) in community/home health or clinical experience preferred.
  • Obtain and maintain current BLS CPR.
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
  • Access to a well working automobile for traveling to patients to provide care.
  • Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.

Occupational Therapist 

Summary of Responsibilities

The Occupational Therapist (OT) will evaluate patients in their place of residence who are recovering from injuries, illnesses, or disabilities and create a plan of care to reach the therapeutic goals of everyday activities as determined by the OT and physician orders. They will use their knowledge and critical thinking to communicate with physicians, clinical supervisors, and other healthcare team members appropriately about changes in patient’s condition to get updates quickly and efficiently to the plan of care. The one-on-one setting ensures that the Occupational Therapist can quickly connect with our patients to guarantee the patient's safe recovery and return to independence while making a positive lasting impression.

Qualifications

  • Possess a master’s or doctoral degree in Occupational Therapy from a nationally accredited school with board approved curriculum.
  • Licensed to practice as an Occupational Therapist in the state of Idaho.
  • One (1) year of experience as a certified Occupational Therapist in community/home health or clinical setting preferred.
  • Obtain and maintain current BLS CPR.
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.

Occupational Therapist Assistant

Summary of Responsibilities

Qualifications

  • Possess a degree in Occupational Therapy Assistant from a nationally accredited school with board approved curriculum.
  • Licensed/ Certified to practice as an Occupational Therapist in the state of Idaho.
  • One (1) year of experience as a Certified Occupational Therapist Assistant in community/home health or clinical setting preferred.
  • Obtain and maintain current BLS CPR.
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
  • Access to a well working automobile for traveling to patients for care.
  • Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.

Speech Language Pathologist 

Summary

The Speech-Language Pathology will use their professional skills to preform diagnostic speech and language evaluations with the patient to develop an individualized plan of care in coordination with the patient's caregiver, physician, and other home health staff. The goal of the Speech-Language Pathology will be to help the patient set therapeutic goals based on their health history and personal concerns that will improve their communication and/or swallowing problems in order to safely to return to independence in the home setting. The one-on-one setting ensures that the SLP can quickly connect with our patients to guarantee patient safe recovery and return to independence while making a positive lasting impression.

Qualifications

  • Hold a master’s or doctoral degree with an emphasis Speech-Language Pathology from a nationally accredited school with board approved curriculum.
  • Licensed to practice as a Speech-Language Pathologist in the state of Idaho.
  • One (1) year experience in Speech-Language Pathology in community/home health or clinical setting preferred.
  • Obtain and maintain current BLS CPR.
  • Capability to work independently and manage time effectively while
  • Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
  • Access to a well working automobile for traveling to patients for care.
  • Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.
