Provide patient name, diagnosis, and
the type of services the patient will require.
*FAX the following information to
(208) 346-7790:
-Order for Home Health or Hospice
-Face Sheet with current demographic and insurance
information.
-History and Physical
-Office Notes (if applicable)
-Medication List (if available)
-Pertinent Laboratory Data (if available)
What to Expect:
-We will contact you if further information is required, and we will arrange a time for admission with the patient and/or family.
-Coordinate all DME, pharmacy and IV infusion needs for each patient.
